How do I share information on my young person's needs?
We’ll have the opportunity to talk through your young person’s needs during our initial assessment phone call, so please feel free to share as much information as you’re comfortable with. We’ll then arrange a home assessment visit, where we can meet your family and discuss things like preferences for support. This visit also gives us a chance to get to know your young person a bit better, so we can match them with specialist support workers who feel like a good fit.
